MEDICAL DOCTOR VISITS AT HOME

Concierge Care of Florida, LLC is a fully mobile medical practice providing home based primary care services in South Florida. We primarily serve South Florida's elderly, homebound patients with medical care in their home, as these services are typically covered under their Medicare health plan and supplemental insurance, however we also provide Concierge Medicine (Doctors on Demand) for private patients and travelers in their home or hotel room based on a fee-for-service model. 

 

When you call us to schedule your visit, you will be sent a link to our HIPAA compliant, secure, patient portal, where you will be able to fill out your consent forms, previous medical history and insurance information. Then we send one of our fully-licensed, board certified physicians directly to your front door for a comprehensive physical examination and assessment.

At the appointment with the doctor, the doctor may prescribe new medications, or refill your currently prescribed medications, if necessary. We can also make arrangements for prescription medication delivery to your home, if needed. If your condition requires bloodwork or imaging services, the doctor will order and schedule a home visit from a local high-tech mobile laboratory, who will draw and collect any specimen, or perform an ultrasound, X-ray, or EKG, in your home.

The doctor will also develop a customized treatment plan, including preventative wellness screenings and can even coordinate care for skilled home health care services, such as: Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Wound Care and many other home service modalities that are appropriate and medically indicated specifically for you.

We coordinate and oversee all aspects of your home care regimen and only work with trusted Medicare-approved Home Health Agencies to provide care within your home. We are a fully-independent mobile medical practice and we will work closely with any of your existing practitioners, including specialists, so that you receive the highest quality of comprehensive care available right from within your own home.

Most (if not all) of these services are fully covered by your insurance plans, in fact, in most cases, we bill your insurance directly. Medicare patients with a supplement often have zero additional out-of-pocket expense to receive most medically-necessary care within their own home. (Your annual deductible requirements, or standard co-pays will still apply.)
