Home Health Care Services are for patients who require rehabilitative care or assistance to live safely and independently in their home. Most medically necessary home health care services ordered by a medical doctor are covered for Medicare patients with a supplement. (Your regular deductible, copay and fees still apply).

Whether you or your loved one needs intensive skilled nursing services, a home care team, physical therapy, occupational therapy, wound care or a home health aide to assist with basic activities of daily living, it is important that you have a physician oversee your plan of care and monitor your progress or make adjustments, as necessary.

While all home health care agencies are independently owned, they must work closely with the ordering physician to follow the plan of care and meet the treatment goals set by your physician.

 

If you are a patient that requires home health care services, during your in home appointment, our doctor will provide you with a comprehensive assessment and create a customized plan of care which will then be carried out by the Medicare-approved home health agency of your choice.
